Please provide the information requested. Print off each form. Fill out Entry Form and Liability Waiver, you may keep the Bash Rules for your records.

Fee is $150.00 which includes:

• 2 T-Shirts

• Dinner

• Awards Ceremony

 

Unregistered guests are welcome for the dinner and awards ceremony for $20.00 each. All entry fees are donated to a local charity.

 

Entry Fees are payable through Go Fund Me or by check.

 

Please make checks payable to this year's Charity. If you are paying your Entry Fee via Go Fund Me, please note that in the comments or in an email so the registration process goes smoothly.
